1st doses, 2nd doses & boosters for ages 5 and above

for HPS Students, Families, Staff & Community!

(2nd doses to be given during the week of 4/18/22 at the same locations.)

Walk-in clinics, no appointments necessary
VISIT ANY SCHOOL ON ANY DAY!
(You don’t need to attend the clinic at your child’s school!)

BRING YOUR WHITE CDC CARD IF THIS IS YOUR 2ND DOSE OR BOOSTER!

On March 29, 2022, representatives from Griffin Health, the CT State Department of Health, the Hartford Department of Health and Human Services, HPS Superintendent Dr. Leslie Torres-Rodriguez, and Hartford Mayor Luke Bronin invited the press to an event at Hartford Magnet Trinity College Academy to promote our Vaccinations Clinics Week at Hartford public Schools.  Watch the entire event below.

Printable Consent Forms and Pre-Vaccination Surveys are below for you to complete.  They will also be available at all the vaccination clinic sites.

For each family member being vaccinated, you must complete a Pre-vaccination Checklist and a Consent Form (Patient Acknowledgement and Attestation Form).

The Pediatric COVID-19 Vaccination Intake Form must also be completed for each child ages 5-11 years old.
