Hartford Public Schools, with Griffin Health, the CT Department of Public Health, and the Hartford Health Department present:

a Free COVID-19 VACCINATION CLINICS WEEK for HPS Students, Families, Staff & Community!

Monday, April 18 – Friday, April 22, 2022 DAILY AT 4:00 Pm – 7:00 pm

  • 1st doses and 2nd doses for ages 5 and above
  • Boosters for ages 12 and above
  • for HPS Students, Families, Staff & Community!
  • PFIZER, MODERNA, AND JOHNSON & JOHNSON
  • Walk-in clinics, no appointments necessary
  • VISIT ANY SCHOOL ON ANY DAY!
    (You don’t need to attend the clinic at your child’s school!)

BRING YOUR WHITE CDC CARD IF THIS IS YOUR 2ND DOSE OR BOOSTER!

Printable Consent Forms and Pre-Vaccination Surveys are below for you to complete.  They will also be available at all the vaccination clinic sites.

For each family member being vaccinated, you must complete a Pre-vaccination Checklist and a Consent Form (Patient Acknowledgement and Attestation Form).

The Pediatric COVID-19 Vaccination Intake Form must also be completed for each child ages 5-11 years old.
