If you have already signed your student up to play soccer or volleyball, please be sure the following have been completed:
Parent forms signed (sent to email on file in FinalForms)
Student forms signed (sent to email on file in FinalForms)
An updated physical is on file with the nurse. Physicals expire in our system after 13 months and students must have an up-to-date physical on file in order to begin participating.
